this data will be stored and maintained under HIPA ( Health Information Privacy Act) compatible server service. Actual data will not be available to any third party. spatial data regarding the diseases will be accessed for statistical purpose.

No personal information ( that includes but not limited to, name, date of birth, address, email, phone number etc) data will ever be distributed nor sold any third party for commercial purpose. Your designated healthcare personell will be able to view your information for the purpose of managing your medical care only. No party who is not involved with your care will not have any access to your information.
